Government health policies are accepted out of network. Please note that all accepted plans are subject to change at any time; likewise, new plans may be accepted.
All owed balances are required at the time of service (including present co-pays and past balances). Our professional staff will inform you of your insurance policy co-pays, deductibles, and coverage percentage before your appointment. Accepted forms of payment are cash, check, and credit card.
